Information You Provide:
- Will be Shared with agencies or persons you decide could be helpful.
- Is used to distinguish you from other clients.
- Enables us to maintain required records for you.
- Assists us in designing services responsive to you.
- Will be shared with persons or agencies for who have signed a release of information.
- Will be shared with your guardian, conservator and/or other legal representatives.
- Will be shared with persons authorized through a Court Order.
Consumer's Rights,
- I have the right to make my own decisions about my life.
- I can withdraw my consent for release of information at any time by placing my request in writing.
- I have the right to my privacy but I understand that by not sharing the necessary information, it limits the agency's ability to help me.
- I have the right to disagree with information that I think is wrong.
Consumer's Responsibilites,
- I will acknowledge my abilities and strengths.
- I will be an active participant in creating my support network..
- I will voice the goals that I have and suggest the expectations I have for services recieved.
- Every client will be responsible for keeping appointments and follow through with goals and objectives set forth.
- In cases when the client feels that they cannot follow a certain program plan or has circumstances that will change the plan set forth, the client will have the responsiblity to advise those individuals involved so that changes can be made and adjusted accordingly.
All of these rights are contigent upon the consumer being the age of 18
